He has no flesh; we only see his skeleton, probably to remind us that after death, nothing physical remains of us either. In his hand, the Grim Reaper sometimes carries an hourglass (as a symbol of the passing of time) or a scythe, to make it clear that ultimately all people will die one day, disappearing from the earth, just like corn stalks being cut down by a farmer.
